RE: HIPAA Business Associate Addendum
April 14th, 2003 is an important date. On this date, the new HIPAA privacy rules become effective. Part of being prepared for this deadline is the finalization of "Chain of Trust" agreements between Covered Entities (healthcare providers) and those organizations that provide services on behalf of the Covered Entity and involve the sharing of Protected Health Information.
Your relationship with Lifeline Systems, Inc., is one that requires such an agreement.
We are asking for your support in speeding compliance with the HIPAA regulation by executing and returning the attached addendum to us before April 1, 2003. Please have the appropriate individual in your organization sign this agreement, retain the original, and send a copy back to us. The attached addendum must be executed for your organization to be compliant with the HIPAA Privacy Regulations.

We would appreciate your cooperation in signing this version of the Business Associate Addendum. It has been drafted based on the guidelines developed by U.S. Department of Health and Human Services, Office for Civil Rights, Standards for Privacy of Individually Identifiable Health Information ¤ 164.504. It has also been reviewed by legal experts on HIPAA compliance and tailored to fit the specific service requirements of Personal Emergency Response Services.
